  • Care Management Programs - team-based, patient focused programs designed to meet specific needs of patients based on the program's criteria.
  • Health Services - a specific focused health services or capabilities designed to meet specific health needs of patients based on the services's criteria.
  • Digital Health Technology - that enables non-medical interventions to be more effective and efficient in meeting specific health needs of patients. These technologies are integrated into Care Management Programs, Health Services and Support Services.
  • Support Services - a specific focused support services or capabilities designed to meet specific needs of patients based on the services's criteria.
  • Patient/Caregiver Interventions - activities or actions by patients and caregivers that will likely improve health, health outcomes and reduce overall cost.
  • Health Insurance Payment Models - these are health insurance payment models design to improve patients outcomes, health and reduce cost
  • Organization Interventions - employers, communities and governments interventions
